Job Summary:
Primary responsibility of this position is to provide care coordination case management services for the BadRiver Health & Wellness Center to coordinate systems of care to provide a holistic healing approach. The Licensed ClinicalSocial Worker (LCSW) is responsible for providing assessment and therapy services to clients of all ages with a variety ofmental health as well as primary care health needs. Establish psychological diagnoses according to the Diagnostic andManual of Disorders. Assess psychological needs and design individualized care plans for each patient including a plan forpsychotherapeutic treatments. Provide individual, group, and family therapy services. Maintain timely completion ofpatient therapy records through the electronic medical record. Complete psychological testing if applicable. Will collaboratewith other professionals to assess case management plans and evaluate follow-through by service providers and clients.
